It's easy, just download and print the Authorization Form, complete the details telling us the amount you wish to pay, and fax it back on: 905.790.3305
You will need Adobe Reader in order to view the pdf file. If you do not have Adobe Reader, you can download it for free from the Adobe website, just click on the link.
If you are having trouble opening the authorization form, "right click" on the link and choose "Save Target As" or "Save Link As" from the drop-down menu.
